Why Do Root Canals Fail?

August 20, 2025

A root canal in Charleston, SC, is a standard dental procedure designed to save a severely decayed, infected, or damaged tooth. It involves removing the infected pulp inside the tooth, cleaning the root canals, and sealing them to prevent further infection.

However, root canals can fail sometimes. Understanding the reasons why this happens can help patients recognize warning signs and seek timely treatment.

Can a Root Canal Fail?

While this treatment has a high success rate—up to 96%—problems can still occur after a root canal. These challenges are often due to the following:

1. Incomplete Cleaning of the Canals

One of the main reasons root canals can fail is that the infection wasn’t removed completely.

Human teeth can have complex root structures; some roots may be curved, narrow, or have extra branches that are hard to detect. If any infected tissue is left behind, it can continue to cause problems.

2. Poor Sealing of the Tooth

After a root canal, the tooth must be properly sealed to prevent bacteria from reentering and causing a new infection. Common sealing issues include inadequate filling material, delayed permanent restoration, faulty crown margins, and material deterioration.

3. Cracks or Fractures in the Tooth

A tooth that has undergone a root canal becomes more fragile and more prone to cracking—especially without the protection of a crown. Cracks allow bacteria to enter, leading to infection and root canal failure.

4. New Decay

Even after a successful root canal, the tooth is still susceptible to decay. For this reason, proper oral hygiene is crucial. You should also let us know as soon as possible if a crown becomes damaged or loose.

5. Complications from Previous Dental Work

Teeth with multiple previous procedures (large fillings, crowns, or prior root canals) can be structurally weakened. These complications can make it more difficult to thoroughly clean and seal the tooth during a root canal, increasing the chances of future problems.

It brings happy tears to my eyes knowing that my smile is fixed. Dr. Jake worked for 2 1/2 hours repairing old, cracked fillings, sealed between my teeth. This weighed on my confidence for three years and I barely ever wanted to talk. My teeth are restored back to their original state and it’s unbelievable what a patient, and kind dentist can do for one’s confidence. I owe ya one, Dr Jake!"
